About CareQED

What CareQED Is

CareQED is an independent, free tool that helps families research senior care using official government records. We cover two kinds of care: assisted living facilities in Florida, using state inspection and enforcement records from the Agency for Health Care Administration (AHCA), and Medicare- and Medicaid-certified nursing homes nationwide, using federal data from the Centers for Medicare & Medicaid Services (CMS).

We are not affiliated with, endorsed by, or operated by AHCA, CMS, or Medicare.gov, and facilities cannot pay to change anything we publish about their record.

Florida Assisted Living: the CareQED Score

For every licensed assisted living facility in Florida, CareQED publishes the facility's full public record: inspection deficiencies by severity class, substantiated complaints, sanctions, fines, licensure details, and services. We summarize each rated facility's record into a 0 to 100 CareQED Score built only from official AHCA findings. The scoring rules are public at /methodology. A facility's Score can only change when AHCA's published record changes. Florida is our first state: we are actively working to expand independent scoring to additional states.

U.S. Nursing Homes: official CMS data

For more than 14,000 certified nursing homes nationwide, we present the official CMS record: star ratings, inspection histories, staffing levels, and deficiency details. CareQED does not score nursing homes; the star ratings shown are CMS's own.

AI-written summaries

The "CareQED Analysis" text on facility and location pages is written by an AI model that is given only the official figures for that facility or area and instructed to summarize them factually. These summaries are generated automatically and are not individually reviewed by a person. They help interpret the data; they never change it.

Data Sources

• Florida AHCA: licensed facility roster, inspection deficiencies, substantiated complaints, sanctions and fines, and licensure records for assisted living facilities.
• Florida Division of Corporations (Sunbiz): corporate filings used to identify facilities under common ownership.
• CMS Provider Data Catalog: Provider Information, Survey Summary, and Health Deficiencies datasets for nursing homes. View the source data.

Each page shows the date of the underlying data. We also publish free datasets and an annual report built from these records at /data.

Records reflect the most recent published government data and may not reflect current conditions. If you believe any information is inaccurate, or you operate a facility and wish to respond to your listing, contact us at support@careqed.com. Facility representatives can also claim a listing from any facility page. Operator responses are displayed but never change a Score. Journalists and researchers can reach us at the same address.
